Participants and Guardians hereby affirm that being involved and participating in Spice Sports Inc, SPICE ON ICE HOCKEY CLINICS, carries a risk of personal injury, including but not limited to; bodily injury, paralysis, disability, and death. I as the participant and/or guardian knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of others.
Participants and guardians understand and agree that neither Spice Sports Inc, nor any members of the program, owners, operators, agents, volunteers, or instructors of Spice Sports Inc may be held liable in any way for any occurrence in connection with the program which may result in injury, death, or other damages to participant, participants family, heirs, or assigns.
I certify that I as parent/guardian with legal responsibility for this participant give my consent for medical treatment in an emergency when I cannot be reached. I understand that medical and personal insurance is the responsibility of the participants.
I also understand that Spice Sports Inc reserves the right to use any photos or videos taken during any program for advertising or promotional purposes. By printing my name below and checking the submit button I have read, fully understand, and agree to this waiver and release of liability terms freely and voluntarily.
